  1. Consent

This is the consent for administering the SCREEN. I need to have your consent before we start the activities. The information in this consent process informs you (1) that the SCREEN is not a diagnostic tool and (2) that the results from the SCREEN are processed through a database that is owned and managed by the creators of the test. This database compares you scores with the other scores so that I can interpret how you did in comparison with the average person. By agreeing, you permit me to administer the SCREEN on you and you allow me to interpret your results. In addition, you permit your scores to be compared with other people’s scores in the Living Brain Bank.


A part of this consent is optional. After you get your results you can choose to donate them to the Living Brain Bank as an anonymous donor. If you choose to donate,


your results aren’t traceable and they will help the test providers increase the accuracy of the Living Brain Bank seeing as you will be one extra person that other test takers will get to compare their results with. In the long term the accumulated data also has the potential to inform future neuroscience research. Please read through both consent forms and ask for clarification on anything that is unclear. Once you have a thorough understanding, decide if you consent to beginning the SCREEN and donating your results and indicate your response on the tablet. Hand client the tablet and stylus.   1. Instructions

The SCREEN has seven activities that are timed. The timer for each activity will appear in the top right hand corner of the screen for most activities. Point. Even though you know that you’re being timed, please try to be as accurate as possible each time that you respond to questions. Accuracy and time count towards the activity score.


You’ll answer each question by touching, sliding, or tracing the touch screen with your finger or with a stylus when directed. At the beginning of each activity, I’m going to listen to you read the instructions out loud. I’ll then ask you if you understand them, this is to ensure that you are prepared for what you’re asked to do once you press the start button. Some activities will have multiple rounds with a “done” button for you to press after each response: in those situations, be sure to select “done” and then click “next”. If you press done but you want to change your answer, just press “resume” to reactivate the touch screen.


There are no breaks between the activities so if you need to use the washroom it’s best to go now.
